Position Summary
The WPPA Nurse Practitioner is responsible for providing primary care services, including assessing, diagnosing, prescribing, treating and educating patients. Works closely with physicians and other clinicians in a team approach to patient care. May specialize in a specific type of clinical care, e.g. pediatrics, geriatrics.
Essential Functions and Responsibilities
- Understands and adheres to the WPH Performance Standards, Policies and Behaviors. Conducts patient rounds as necessary. Formulates and implements a therapeutic plan of care with physicians and staff. Writes in progress notes.
- Assesses daily plan of care and recommends actions in collaboration with the patient's physician.
- Orders and interprets appropriate laboratory and diagnostic testing. Maintains contact with the ancillary testing areas for follow up.
- Performs regular medication reviews for patients and modifies orders as necessary.
- Recommends appropriate consults as needed.
- Communicates with patient / family regarding plan of care and discharge plan.
- Interacts with third party insurers regarding plan of care and continued stay.
- Participates as needed in finalizing a safe discharge plan. Completes all required documentation.
- Assists staff in moving patient along clinical pathway. Serves as a resource to the discharge planning staff.
- Inpatient rounding – assist physicians with pre-op and post-op patient assessments.
- Assist physicians as requested with appropriate surgical procedures.
- Performs all other related duties as assigned.
Education & Experience Requirements
Master's degree in nursing from an accredited school.Current NYS license to practice as a Registered Professional Nurse.Current NYS certification as a Nurse Practitioner (either Adult / Geriatric).Maintains a current collaborative practice agreement.BCLS certification required. ACLS certification required as of 6 / 1 / 21 for NPs / PAs in CAMS & Cancer Center.Minimum two years' experience in an acute care setting.Core Competencies
