Your Responsibilities as a Home Health Nurse :

Deliver home-based care to patients recovering from illnesses, injuries, and chronic diseases, often post-hospital discharge, under RN supervision

Collaborate with a dedicated team of physicians, RNs, CNAs, and aides to ensure quality patient care

Monitor patient vitals, conduct assessments, document progress, and report changes to your RN supervisor

Assist RNs in implementing patient care plans and teaching self-care techniques

Educate families on the patient’s care plan, medications, and home safety

Work with an interdisciplinary team to coordinate patient care and plan for discharge

Must-Haves for Home Health Nurses :

Active, unrestricted Licensed Practical Nurse (LPN) license in the State of Michigan

At least one (1) year of clinical experience as an LPN

Current CPR Certification

Reliable transportation with a valid state driver's license and current car insurance
