Position Title : PT I Total number of positions needed : 1 REQUIRED : I CANNOT post without the ICIMS # in the system for the perm posting : _______________ 1. How long is the contract assignment? 13 weeks 2. Schedule of assignment hours? M – F, 8am – 5pm 3. Will this candidate be visiting patients in care facilities or only in patient’s homes? Yes 4. Will you want to hire the contractor at the end of the contract or is this truly a contract assignment? Possibly 6. What region are you in? Home health or Hospice? Home Health Towns and Union County 7. What’s creating the request? - work load 8. To which location will this person report for assignment, full address, Including the OU (Location Code) 417 Blue Ridge Street Blairsville, GA 30512 Please confirm above location is correct and list any other territories that are to be covered by the candidates : 9. How many miles per week will the consultant travel? 30 / 40 a day 10. What are the weekly units of productivity expectations? 25 / 30 11.
