As a Chaplain You will :
Conduct and ensure timely spiritual assessments for patients and familiesParticipate in developing individualized care plans to address spiritual needsProvides direct spiritual support to patients, families, and caregiversServes as a liaison to community chaplains and spiritual counselorsMaintains accurate documentation of spiritual services and communicationsAttends interdisciplinary meetings and contributes to care planningOffers education and spiritual care guidance to the care teamRecruits and supports community chaplains, spiritual counselors, and volunteersCoordinates and leads memorial or funeral services as requestedOrganizes periodic memorials for staff, volunteers, and community partnersAssists in supervising spiritual care volunteersQualifications
Graduate of an accredited theology or chaplaincy program (BD, MDiv or equivalent theological degree)Experience working with death and dying, hospice experience preferredComfort in an interfaith settingMust be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working orderAbility to accept different lifestyles, cultures, beliefs and valuesKnowledge of and commitment to hospice philosophy of careAbility to work as member of interdisciplinary group
