Licensed Practical Nurse

About the Role : Talent4health is seeking a “Licensed Practical Nurse (LPN)” contract position for a facility in Ithaca, New York, United States . Responsibilities : Experience in rehab or mental health preferred, any outpatient center exp will be considered-attention to detail, dealing with controlled substances, cannot make any medication errors. LPN will give dosage to patients, needs to have empathy since they are dealing with unique patients, most patients use the facility as their “primary care”, so they do general wellness checks with them. Give liquid methadone, will do other injectable medications from time to time. Perform routine wound care, including dressing changes and treatment of bedsores. Collaborate with the healthcare team to develop and implement individualized care plans. Monitor and record vital signs, including temperature, blood pressure, pulse, and respiration rates. Provide guidance and supervision to Certified Nursing Assistants (CNAs) and other support staff. Work closely with RNs, physicians, and other healthcare professionals to ensure coordinated and effective care. Uses Epic for charting but uses Evolve for dosing. Qualifications : Education : High school diploma, GED certification or equivalent or greater – Required. Required Skills : Minimum 2 years' experience required. License REQ : An active NY State Licensed Practical Nurse is required. Certs REQ : BLS - Basic Life Support.
