Job Description As a physical therapist, you will be responsible for assessing patient needs and developing comprehensive treatment plans that promote optimal health and well-being in a home care setting.

You will conduct thorough initial and ongoing assessments of patient condition to determine appropriate treatment plans.

Administer physical therapy treatments as prescribed by physicians, ensuring the highest level of care and comfort for patients.

Evaluate treatment outcomes and make recommendations for adjustments or modifications as needed.

Collaborate with multidisciplinary teams to ensure seamless transitions and continued care for patients after discharge.

Qualifications 1+ years of experience in physical therapy, preferably in an acute or post-acute care environment.

Current state licensure and valid CPR certification.

Valid driver's license.
