Role Overview :
Nurses play a critical role in the successful management of individuals within the ACT model. The ACT RN assumes the lead in filling prescription orders, storing and putting together medication packets, and coordinating medication deliveries. The ACT RN ensures that the Medication Administration Record (MAR) and all other documentation related to medications are accurate and up to date. The ACT RN manages the intramuscular (IM) injection schedule and administers injections both in the community and in the office. In collaboration with the Transdisciplinary Treatment Team, the ACT RN assists participants in developing strategies to maximize the taking of medications as prescribed.
The ACT RN conducts regular screenings and assessments. These may include screenings for medical conditions, medication side effects (i.e., conducting the Assessment of Involuntary Movement (AIMS) Screening), and monitoring of existing medical conditions. The ACT RN tracks age-related and family history health screens (i.e., mammograms for women over age 40, and prostate exams for men over age 50). In addition, assessments for risky behaviors (i.e., unsafe sex practices, unsafe needle usage, or needle sharing) will be conducted by the ACT RN. It will be important for the ACT RN to work with participants on behavior change and / or ways to reduce the potential for harm. The ACT RN assesses and intervenes on health / medical risk factors or conditions by providing services such as education to the participant and their designated natural supports (i.e., monitoring diabetic needs, preventing and / or addressing obesity, hypertension, and high cholesterol).
The ACT RN has regular contact with inpatient and outpatient medical and psychiatric care providers for participants to coordinate care. The ACT RN may take participants to doctor appointments, especially when the participants' condition(s) and / or care needs are complex.
The ACT RN utilizes creativity to promote health and wellness while helping participants with basic needs. Examples include providing diabetes education while helping a participant go shopping, walking with the participant to promote health and wellness, or assisting the participant in developing strategies to reduce tobacco use.
The ACT RN attends and participates in all daily team meetings and treatment planning meetings for those participants on their individual treatment team (ITT). The ACT RN provides cross-training to other team members to help them monitor participants' psychiatric symptoms and medication side effects. When the ACT RN serves in the role of "primary" case manager, they take the lead in compiling the integrated assessment, team case conceptualization, and treatment planning.
Role Standards / Certifications :
The ACT nurse must be licensed as a Professional or Registered Nurse in the State of Idaho as defined by Idaho Code 54-1408 or state licensure as a Registered Nurse pursuant to the Nurse Licensure Compact, Idaho Code 54-1418. They must also have their Risk Evaluation Mitigation System (REMS) certification and the Psychiatric Mental Health Nurse Certification (PMH-BC), which is optional but recommended.
To be an ACT nurse in Idaho, he or she must complete the Idaho ACT Registered Nurse Training Program within six months of being hired on. This training is an initial 12-hour training compiled of knowledge in medication management, assessments, coordinated care, interventions, health promotion, case management, supervision, support, and profession development.
