Home Instead® is looking for a caring and compassionate  Registered Nurse (RN)  to join our wonderful team as a  per diem   Director of Patient Services  and join our mission of enhancing the lives of aging adults throughout the community!

Home Instead® provides a variety of home care services that allow seniors to remain in their homes and meet the challenges of aging with dignity, care, and compassion. The Director of Patient Services at Home Instead will be responsible for overseeing all aspects of patient care within a home health agency, ensuring high quality care and compliance with regulations. This role involves managing the agency's day-to-day operations, including clinical operations, quality assurance, and staffing. The director also plays a crucial role in developing and implementing policies and procedures to maintain quality standard

Qualifications :

  • License and current registration to practice as a Registered Professional Nurse in New York State
  • Graduate of an school of nursing and holds a Diploma, Baccalaureate or Masters Degree
  • Home care experience in a licensed home care agency preferred

Primary responsibilities include, but are not limited to :

Clinical Operations Management :

  • Monitoring and assessing the delivery of home health field services.
  • Overseeing and directing the clinical aspects of patient care by providing case management support, clinical intervention, development and implementation of care plans and follow up as needed. Works under general supervision.
  • Collaborating with the primary care physician to perform a full patient assessment by gathering the patient's medical history and medication information.
  • Delivering the start care packet and providing an overall review with the patient / caregiver on initial visit.
  • Quality Assurance :

  • Ensuring that all patient care services meet established quality standards and regulatory requirements.
  • Performing initial and ongoing supervisory field visits to patients' homes to provide assessment, training support, in-home coordination and / or crisis management.
  • Conveying any significant changes in the patient's condition, emergency intervention, or plan care changes.
  • Reporting all interventions and document findings.
  • Participating in quality assurance and education programs as requested by management.
  • Staff Management :

  • Supervising and supporting the agency's clinical and administrative staff, including training and performance evaluation.
  • Overseeing the initial home home patient to ensure the HHAs and PCAs are following Home Instead guidelines and regulations.
  • Identifying continued training needs of the HHAs and PCAs and documenting the information.
  • Educating and reviewing the paraprofessional plan of care with the HHAs and PCAs.
  • Policy and Procedure Development :

  • Participating in the development and implementation of agency policies and procedures related to patient care, quality, and operations.
  • Regulatory Compliance :

  • Ensuring that the agency is compliant with all applicable state and federal regulations.
  • Secondary Responsibilities :

  • Ability to communicate with clients and employees in a friendly, caring, and congenial manner
  • Complete and meet the requirements of a criminal background check, motor vehicle record check, sex registry screen, and drug screen
  • Ability to operate Microsoft PowerPoint, Excel, and Word programs
  • Possess a valid driver’s license and valid auto insurance
