UPMC Family Hospice Home Care Aide
UPMC Family Hospice is hiring a Full-Time Home Care Aide for the State College and surrounding areas. Hours are Monday-Friday 8 : 00 am-4 : 30 pm.
Responsibilities include :
- Communicating appropriately with the interdisciplinary home care team promptly, reporting any changes in patients' condition to appropriate professional staff.
- Attending compliance training and adhering to the organization's standards of conduct, policies, and procedures.
- Focusing on customer service and continually striving to perform the duties of their job in a manner that will result in optimal patient satisfaction.
- Providing safe and direct personal care to patients in the home setting under the direct supervision of an appropriate professional.
- Performing duties and job responsibilities in a fashion that coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians, and other departments within the organization.
- Identifying, developing, and participating in process improvement opportunities within the home health agency that will enhance the quality of services we provide.
- Proven ability to facilitate change.
- Maintaining and completing documentation accurately and promptly while in the patient's home and within the agency and regulatory standards.
- Participating in all required agency meetings.
- Maintaining annual competency evaluation scores as determined by regulatory requirements.
Qualifications :
High School Diploma or GED - RequiredOne (1) year of direct care experience - requiredCertified Nurse Aide Preferred. Minimum of 75 hours of Nurse Aide training required. If no previous nurse aide training, applicant may attend UPMC Home Healthcare Nurse Aide Training Course as available upon hire.Licensure, Certifications, and Clearances : Preferred : Nurse Aide Registry in good standing, either in the state where the facility is located or in a state covered by a licensure compact agreement with the state where the facility is located.
