Nurse Practitioner NP

RiteMed Group is seeking a dedicated and compassionate Nurse Practitioner to join our provider-led team at Family of Caring at Park Ridge, a respected skilled nursing facility in northern New Jersey. This Part-time role is perfect for an NP who values autonomy, clinical support, and making a daily impact in post-acute care.

What You’ll Do :

Provide primary and acute care to residents within the facility

Conduct assessments, manage chronic and acute conditions, and help reduce avoidable hospitalizations

Collaborate with the facility’s nursing team and interdisciplinary staff

Communicate with families, specialists, and discharge planners as needed

Maintain accurate and timely documentation

Requirements :

Active Nurse Practitioner license in New Jersey

Minimum of 6 months NP experience (ICU or SNF background a plus)

Strong clinical decision-making and interpersonal skills

Comfort working independently in a long-term care setting
