Job Summary :
Provide comprehensive psychosocial assessments, interventions, and discharge planning services to patients and families while actively participating in hospital quality improvement initiatives. This dual-role position combines direct patient care with data analysis and process improvement activities to enhance patient outcomes and satisfaction.
Essential Functions :
Clinical Social Work Responsibilities :
Conduct comprehensive psychosocial assessments to identify patient and family needs, barriers to care, and discharge planning requirementsProvide crisis intervention, counseling, and emotional support to patients and families facing acute medical situationsCollaborate with interdisciplinary teams to develop and implement treatment plansFacilitate family meetings and care conferencesCoordinate discharge planning, including placement in appropriate care facilities, home health services, and community resourcesProvide education and support regarding advance directives and end-of-life planningAssess for abuse, neglect, or safety concerns and initiate appropriate interventionsMaintain accurate, timely documentation in compliance with regulatory standardsQuality Initiative Responsibilities :
Participate in quality improvement projects focused on patient experience, readmission reduction, and care transitionsCollect, analyze, and report data related to social work interventions and patient outcomesIdentify trends and patterns in discharge delays, patient satisfaction scores, and care coordination challengesCollaborate with quality department to develop and implement process improvementsParticipate in root cause analysis for quality events involving psychosocial factorsSupport development of evidence-based protocols for social work practiceAssist with regulatory survey preparation and compliance monitoringContribute to interdisciplinary quality committees and patient safety initiativesRequirements :
Required Qualifications :
Master's degree in Social Work from CSWE-accredited programCurrent state licensure (LCSW, LMSW, or equivalent) LCSW HIGHLY PREFERREDMinimum 2 years of healthcare social work experiencePreferred 1 year experience in hospital settingKnowledge of healthcare regulations and accreditation standardsStrong analytical and data interpretation skillsProficiency in electronic health records and quality management softwarePreferred Qualifications :
Previous experience in quality improvement or process improvement initiativesLean Six Sigma or similar quality methodology certificationExperience with discharge planning and care transitionsKnowledge of HCAHPS, CMS quality measures, and value-based care conceptsKey Competencies :
