Must-Have :

• Previous charge experience
• Patient ratio experience
• Experience with charting systems, specifically Epic
• Experience in acute hospital and community hospital settings
• Travel experience required

Description :

Floating expectations across facilities are required.

All time-off requests should be submitted upfront.

Hospital experience is required.

Case Managers will not be involved in utilization review.

Experience in assessing patients, care coordination, and discharge planning is essential.

Candidates should be able to organize and participate in family meetings.

Participation in the development of treatment plans is expected.

Experience with managed care organizations and integrated care systems is preferred.

Knowledge of adoption and foster care processes is beneficial.

Understanding of drug and alcohol abuse treatment options is necessary.

Competency in handling cases involving homelessness, mental health history, and crises is needed.

Knowledge of advance directive processes and protective services is important.

Ability to arrange durable medical equipment and transport services independently is required.

All travelers are expected to float between facilities within the state.

Refusal to float may result in termination of assignment.

Communication devices will be provided by the employer or reimbursed for work-related usage.

Modules are non-billable and must be completed annually.

Module completion time is self-reported and recorded.

A commitment to work three of five major holidays is necessary.

References should include a minimum of two supervisory contacts covering one year within the last three years in a comparable specialty or setting.
