TNCSA is a provider of Independent Support Coordination (ISC) services for the Department of Disability and Aging (DDA). The Independent Support Coordination Program assists individuals with intellectual and development delays to live as independently as possible within their communities through the provision of community-based services.
About the Role
The role involves providing case management services and support to individuals with developmental and intellectual disabilities.
Responsibilities
- Case management services for individuals with developmental and intellectual disabilities.
- Conduct meetings, writing individual support plans and submitting those plans to DDA to request services.
- Monthly monitoring of services and completion of vital paperwork.
Qualifications
Must live in the Memphis / Shelby County area due to required client visits.Bachelor's degree – Preferred field social work, psychology, sociology or related field.One to two years of work-related experience.ISC or DDA experience and / or certification a plus.Excellent written and verbal communication skills.Must be very organized and detail oriented.A valid motor vehicle operator's license is required. Must have reliable transportation and current automobile insurance.Must authorize the release of any investigative and criminal records obtained by the Federal Bureau of Investigation and the Tennessee Bureau of Investigation to the Tennessee Community Services Agency.Required Skills
Excellent written and verbal communication skills.Organizational skills and attention to detail.Ability to manage casework effectively.Valid Drivers License.Dependable transportation.Preferred Skills
