The Physical Therapist will be responsible for assessing, planning, and implementing individualized therapy programs for residents in a skilled nursing facility. The role involves working closely with a multidisciplinary team to deliver high-quality rehabilitative care that promotes functional independence and overall well-being.

Responsibilities :
Patient Assessment : Evaluate patients’ physical conditions, mobility, and functional abilities to develop comprehensive treatment plans tailored to their needs.Treatment Planning : Create and implement personalized therapy programs that address specific goals, improve mobility, alleviate pain, and enhance overall physical function.Therapeutic Interventions : Administer therapeutic exercises, manual therapy techniques, and modalities to help patients achieve their rehabilitation goals.Progress Monitoring : Regularly assess and document patient progress, adjusting treatment plans as necessary to optimize outcomes.Patient Education : Educate patients and their families on exercise techniques, safety measures, and home care strategies to support ongoing recovery and maintenance.Collaboration : Work closely with physicians, nurses, and other healthcare professionals to ensure coordinated care and address any issues that may impact patient progress.Documentation : Maintain accurate and timely documentation of patient evaluations, treatment plans, progress notes, and discharge summaries in compliance with facility policies and regulatory requirements.Compliance : Adhere to all safety and infection control protocols, as well as facility policies and procedures, to ensure a safe and effective therapeutic environment.Qualifications :
Education : Doctoral or Master’s degree in Physical Therapy from an accredited institution.Licensure : Current state licensure as a Physical Therapist.Experience : Previous experience in a skilled nursing or long-term care setting preferred, but not required.Personal Attributes : Compassionate, patient-focused, and dedicated to providing high-quality care.
