We want you to apply!\n\nJob Summary And Qualifications\n\nThe Case Manager (CM) ensures high-quality, patient-centered care by managing Rehabilitative care to ensure optimum outcomes. The CM provides, coordinates, and directs care specific to the needs of each Rehab patient. The CM collaborates with the Rehab Program Director and Facility Case Management Director regarding departmental functions. The CM coordinates efforts within the Rehabilitation team.\n\nWhat qualifications you will need : \n
• (RN) Registered Nurse\n
• Associate Degree, or Bachelors Degree, or Masters Degree, or Registered Nurse Diploma\n
• RN with current state licensure; BSN preferred\n
• Three years' clinical, hospital nursing experience required with preference given to those with 2 years of case management experience\n
• or 2 years of critical care experience\n
• Certification in case management or utilization review preferred\n
• InterQual experience preferred\n
