Clinical Pharmacy Intern
TO PERFORM CLINICAL PHARMACY INTERN ACTIVITIES UNDER THE DIRECT SUPERVISION OF A LICENSED PHARMACIST EMPHASIZING MEDICATION RECONCILIATION AS A KEY AREA IN LEARNING TO BECOME A PHARMACIST AND PROVIDING ASSISTANCE WITH PHARMACY OPERATIONS ACTIVITIES WHEN NEEDED.
1.
Perform medication reconciliation activities including interviews with patients admitted to Holyoke Medical Center with appropriate documentation as required.
2.
Provide pharmaceutical care including but not limited to : pharmacokinetic and renal drug dosing and monitoring, anticoagulation monitoring, and conversion of IV to oral medication.
3.
Use the EMR to retrieve patient and medication information and manage home medication information.
4.
Provide drug information to other health care professionals
Drug Distribution Activities
1.
Perform distribution activities to support patient care, as needed. Activities may include (but are not limited to) : drug delivery, restocking bins and automated dispensing cabinets, unit dose packing of oral medications, inventory maintenance, customer service, answering and directing telephone calls and clerical functions.
2.
Maintenance of drug distribution knowledge and Pyxis operation is expected.
Other Activities
1.
Contribute to the mission of the organization by following policies and procedures and adhering to the Ethical Code of Conduct and Corporate Compliance Program. Abide by laws and regulations that govern pharmacy intern activities.
2.
Complete required competencies as required by the institution and department.
3.
Perform other activities as requested by direct pharmacists’ supervisors or pharmacy administration.
