What You ll Do :

Provide comprehensive care for patients with chronic kidney disease, hypertension, and electrolyte disorders

Manage patients on dialysis and coordinate care with transplant teams

Work with primary care providers and specialists to optimize kidney health and patient outcomes

Engage in patient education, quality initiatives, and collaborative treatment planning
