Job Summary
Manages individualized, goal-directed nursing care in accordance with departmental and hospital policies and procedures. Develops and implements plan of care and ensures quality assurance. Makes initial and ongoing assessment of the patient needs, identifies changes and trends in various physical and emotional parameters to ensure expected outcomes. Provides clinical leadership and expertise in a specialty. Facilitates excellence in nursing practice through mentoring and education at the bedside, staff meetings, and select committees. Supervises and directs the activities of various levels of assigned nursing staff and coordinates care with ancillary personnel utilizing professional and supervisory discretion and independent judgment.
Job Requirements
Education and Work Experience :
Master's Degree in Nursing : Required
Two years' clinical nursing experience : Preferred
Licenses / Certifications
Registered Nurse (RN) licensure in the state of practice : Required
Clinical Nurse Specialist (CNS) in the state of practice : Required
Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS OR HS-BLS OR RQIBLS) certification : Preferred
Facility Specific License / Certifications
Hospital Fire and Life Safety (HLFS) : Required
Essential Functions
Develops a prioritized interdisciplinary plan of care, which is individualized, updated and documented, on each patient to guide the delivery of nursing care. The plans include the developmental needs of the age-specific population served and include the patient in this planning process. Revises the plan to meet changing needs of patients assigned. Collaborates with physicians in the diagnosis, evaluation and management of surgery patients. Performs routine follow-up care after review of patient care needs, determining the daily plan of care for each patient. Reviews patient progress throughout the day with adjustment of the care plan.
Communicates with patients including answering questions, scheduling and / or doing consults, scheduling surgery and overnight stays, ordering implants and surgical garments, and providing information to patient on recovery. Performs pre-operative history and patient education. Identified patient / caregiver learning needs and assesses associated knowledge, skill level and readiness for learning.
Participates in discharge planning with patients, physicians, social workers, patient education, utilization review and other personnel as appropriate.
Consistently documents pertinent information based on changes in patient condition / response and on the care given, as outlined in the patient care plan. Completes all nursing forms accurately and in a timely manner. Transcribes and notes physician orders accurately.
Participates in the educating the medical and nursing staff.
Performs other job-related duties as assigned.
