About The Position

Job Title : Pharmacist Inpatient

Profession : Pharmacy

Specialty : Pharmacist Inpatient

Duration : 26 weeks

Shift : Night 4x10-Hour

Hours per Shift : 10 hours

Experience : Minimum 2 years in specialty preferred

License : Pharmacist license in the state of practice required

Certifications : Registered Pharmacist (RPH) required

Must-Have :

• Graduate of school of pharmacy required
• Doctor of Pharmacy (PharmD) degree preferred
• One year's technical experience preferred
• Inpatient clinical pharmacy experience preferred

Description :

Verify and fill orders for medications.

Monitor patient medication therapies.

Provide drug information.

Identify and troubleshoot potential medication safety hazards.

Document all interactions for medical review.

Supervise and direct support personnel.

Process, enter, verify, and distribute medication orders in a timely manner.

Ensure compliance with department and hospital policies as well as regulatory guidelines.

Actively ensure medication therapy is efficacious, safe, and cost-effective.

Assure optimal medication prescribing concerning indication, dose, route, frequency, and allergy history.

Conduct and manage daily reports on medication utilization based on daily rounds.

Identify and analyze areas needing medication improvement.

Perform quality assurance procedures in related work areas.

Participate in pharmacy-related tasks and programs such as clinical rounds and medication management activities.

Expedite changes needed to meet regulatory standards and laws.

Participate in department cost savings.

Comply with drug formulary and appropriate documentation.

Collaborate with leadership and buyers to procure medications and supplies based on medication safety standards.

Assist in recommendations for process improvement or the development of new pharmacy services.

Perform other job-related duties as assigned.

Adhere to safety and wellbeing requirements for associates and patients.
