SOUTHEAST COLORADO HOSPITAL DISTRICT JOB DESCRIPTION DEPARTMENT : LTCC / ALZHEIMER S UNIT NURSING POSITION TITLE : MEDICATION NURSE REPORTING RELATIONSHIP : RESIDENT CARE COORDINATOR, ADON, DON SUPERVISES DIRECTLY : CNA s, Restorative CNA s, Support Staff PURPOSE FOR Is responsible to provide direct and indirect total nursing care of THE POSITION : residents during the assigned shift of work. Responsible for the direction and supervision of CNA s performing resident care. ESSENTIAL DUTIES & RESPONSIBILITIES : Assigns, directs, and supervises resident care; addresses resident needs or concerns as it occurs and enters on nursing care plan; administers medications and treatments as ordered by physician; reports changes occurring with residents or personnel to Resident Care Coordinator; keeps physician and families informed about residents status as needed; promotes teamwork between staff members, shifts, and other departments. Completes required documentation accurately and in a timely manner. May work all shifts as Treatment Nurse as need arises. When an LPN and RN are concurrently working the same shift, the RN will assume role of Charge Nurse and the LPN will report to the RN. Attends meetings and educational sessions, as directed. Regular attendance and punctuality. Other duties as assigned. MINIMUM QUALIFICATIONS : Current Colorado R.N. or LPN license in good standing. CPR certificate. One year of Staff or Charge Nurse experience in Long Term Care is desirable. MINIMUM PHYSICAL REQUIREMENTS : Using the percentage range as listed : 0% = Never; 1% - 33% = Occasional; 34% - 66% = Frequent; 67% - 100% = Continuous. Requires full range of body motion, including : 1. Standing / Walking : Frequent. Worker is on feet the majority of the day to perform direct or indirect resident care and monitoring functions of this job. 2. Bending / Stooping : (Knees extended; standing with knees flexed while standing; sitting) : Frequent. Bending / Stooping is required in most job activities (making beds, emptying catheters, bed pans, obtaining linens, and repositioning residents). 3. Lifting / Handling : Frequent. Range of weight 10-30 pounds. Resident may weigh approximately 100-300 pounds. Assistance is available if resident needs to be moved from bed to wheelchair. If the resident collapses to the floor, the worker must be able to lift the resident with assistance from floor level to chair. This activity is frequently done with push / pull activity. 4. Carrying : Occasional. Range of weight is 1-25 pounds. Worker may carry food trays, medication, linen of various weights, and boxes of charts. 5. Push / Pull : Frequent. May perform push / pull motion when maneuvering medicine carts, moving beds and wheelchairs intermittently over concrete / tile floor or carpeted floors. Push / pull activities are also required throughout the day with resident positioning, linen changing, and bathing. Weights vary from 25-250 pounds with a variant frequency of activities. 6. Balancing : Occasional. May be necessary when the worker is assisting with ambulation or in the event of unexpected movement from resident. 7. Twisting / Turning : Frequent. Spent in turning activities in combination with lifting and bending. The employee may twist when stocking various cabinets, maneuvering residents, assisting residents from chair to bed, reading vital signs, dressing changes, administering medications, bathing, measuring and emptying catheters, and performing CPR.. 8. Crouching / Stooping : Occasional. Crouching is necessary for a variety of resident care activities such as emptying catheter bags, positioning residents, obtaining trays out of carts, obtaining medicine through medicine room, bathing and / or dressing residents. 9. Kneeling : Occasional. 10. Reaching : Occasional. a. Greater than shoulder height : Occasional when worker reaches for supplies from storage shelves, etc. b. Equal to shoulder height : Occasional when worker reaches for charts, supplies from storage shelves, etc. c. Less than shoulder height : Occasional when worker reaches for supplies from storage shelves, charts, etc. 11. Handling / Manual Dexterity : Continuous. Both fine and gross motor skills are needed in all types of resident care activities. 12. Speaking / Hearing / Seeing : Continuous. a. Speaking : Communicating verbally. b. Hearing with or without correction : Continuous. Within normal range of 0-2 decibels. c. Seeing with or without correction : Continuous. Within normal range of 20 / 30. OTHER REQUIREMENTS : Must be professional, flexible, caring, and compassionate and exhibit a congenial and sensitive attitude toward residents. Must have a positive attitude. Ability to establish and maintain effective working relationships with residents, their families and staff members. Requires working under stressful conditions and / or irregular hours. Requires exposure to communicable diseases and / or body fluids. Frequent exposures common to clinical environment such as medical preparations and toxic substances. Occasional exposure to ionizing radiation. JOB LOCATION : Long Term Care Center or Alzheimer s Unit as assigned. The floor surfaces are tile and / or carpet.
