Role at a glance

You’ll lead community outreach and resource tabling focused on HIV prevention and harm reduction, provide HIV testing and linkages (PrEP / PEP / Doxy-PEP), and enroll neighbors into LIW programs. You’ll also keep clean, timely data so our team can move quickly for clients.

What you’ll do

Conduct HIV prevention education; distribute Narcan, condoms, lube, and safer-use materials.

• Provide HIV testing; make warm linkages to HIV care, PrEP / PEP / Doxy-PEP, STI testing / treatment, and gender-affirming care.
• Coordinate outreach events and represent LIW at community tables / health fairs.
• Complete intakes and referrals; document services in CMR and support case conferencing (HIPAA compliant).
• Help clients access wraparound supports (legal, mental health, nutrition / solidarity closet).
• Join required trainings (IDPH, CaseWorthy / CMR, data privacy, mandated reporting).

What you bring

Lived experience and / or AA / BA in Social Work, Public / Community Health, or related field.

Work style & schedule

Mostly daytime with some evenings / weekends for outreach; supportive team culture; ongoing training and growth.
