Our client is seeking a compassionate Medical Social Worker to join their Home Health team in Lansing, MI. The ideal candidate will provide emotional, social, and resource support to patients and families while collaborating with the interdisciplinary care team to ensure quality home-based care.

Responsibilities include developing and implementing patient care plans, providing support to patients in Lansing and surrounding communities, maintaining accurate and timely documentation, managing a part-time caseload, and traveling locally as needed.

Schedule : Flexible part-time hours, Monday Friday

Travel : Local travel required (~20% of time)

Required qualifications :

Master's degree in Social Work from an accredited program (CSWE-approved)

One to two years of post-graduate experience in a healthcare or home health setting

Active MSW license in the State of Michigan

Valid driver's license, reliable transportation, and auto insurance

Proficiency with EMR systems such as TruChart

Strong organizational, communication, and time management skills
