Join our team and earn a $5,000 bonus – paid in two parts :
- 50% at 6 months of employment
- 50% at 1 year
- Bonus available to external / new hires only
Office Location : 1028 S. Main Street Suite C Fond du Lac, WI 54935
Coverage area : Travel throughout the territory is required
Schedule : Monday-Friday 8AM-4 : 30PM w / On Call Responsibilities

Visit patients (in homes, hospitals, nursing facilities, etc.) who are referred for hospice careExplain hospice services, discuss goals and care with the patient and families, answer any questions, and obtain necessary consents and legal documentsObtain medical history, current medications, and symptomsPerform comprehensive clinical assessments to determine eligibility for hospice based on Medicare / insurance guidelinesCoordinate the start of services (nursing, aides, equipment, medications.)Collaborate with interdisciplinary teamMaintain accurate and timely documentationBenefits and Perks for You!

Graduate of an accredited nursing school with current licensure to practice in state of operationOne year of hospice nursing experience required, 3+ years preferred or similar setting such as Long-Term Care, Intensive care, or Emergency room will be considered in place of hospice nursing experienceMust be a licensed driver with an automobile that is insured in accordance with state / or organization requirements and is in good working order
