Education : Bachelor’s Degree and / or a Pharm. D from an ACPE accredited institution Experience : A minimum of 3 years’ hospital experience as licensed Pharmacist in a hospital setting.

Certifications : If specifically going with a BCOP – Board Certified Pharmacotherapy Specialist or Board Certified Oncology Pharmacist.
