You'll partner with an interdisciplinary team to develop individualized care plans, guide end-of-life conversations, and ensure comfort with professionalism and empathy.\n\nIf you're a compassionate Registered Nurse who values meaningful patient connections and autonomy in your practice, we'd love to meet you.\n\nRegistered Nurse Case Manager Requirements & Responsibilities : \n

• Associates, Bachelors, or Masters of an accredited nursing program (RN)\n
• 1+ years of recent acute care experience required; hospice / home health experience preferred\n
• Complete initial and ongoing comprehensive assessments to determine hospice needs\n
• Develop individualized care plans with the patient and family, updating as needs evolve\n
• Administer medications and treatments as prescribed\n
• Educate patients and caregivers on care techniques, medications, diet, and symptom management\n
• Rotate on-call duties and ensure seamless care transitions\n
• Must have reliable transportation, valid driver's license, and proof of auto insurance\n\nBenefits & Schedule : \n
• Schedule : 8 : 00am to 5 : 00pm, Full-time position with on-call expectations\n

• Paid time off and paid holidays\n\nThe starting salary range for this role is $75,000-$101,000 annual base salary. An employee's actual annual salary will be based on but not limited to : geographic location, relevant education, applicable certifications, depth of experience, and special skillsets. The total compensation package for this position may also include the opportunity to participate in employee benefits including our medical / dental / vision / pet insurance, disability and life insurance, paid time off, and 401(k) retirement plans, subject to applicable plan terms, and / or eligibility for other compensation.
