Must-Have :

Ensure prescreen is filled out and attached upon submission of candidate.

Resume and license verification must also be attached.

Covid test required 72 hours prior to start date with results issued 48 hours prior to start date.

Description :

Candidates will be considered if the following checklist is completed prior to submission.

State license verification is necessary.

A current resume must be provided.

Three references are required.

County criminal background check for the last 7 years for all counties lived in or worked in.

Federal criminal background check for the last 7 years or more.

Multi-state criminal search for the last 7 years or more.

Nationwide sex offender search for the last 7 years or more.

HHS / OIG search must be completed.

GSA search is necessary.

Social security number trace is required.

Two-step PPD or Quantiferon test or proof of positive PPD and chest X-ray documentation must be available.

SAM search needs to be conducted.

Iowa SING background check is required.
