Nurse Practitioner Flushing, Queens  (must be willing to attend 2-day orientation in the Bronx)

Schedule : Monday – Friday | 8 : 00 AM – 5 : 00 PM (1-hour lunch) | Full-time or Part-time

Setting : In-person clinic + some field / home care

Assignment Length : Open-ended

Required Qualifications :

Active Nurse Practitioner license in NY

Minimum 3 years’ experience in Geriatrics, Primary Care, or Internal Medicine

DEA License and active Medicare & Medicaid billing IDs

Willingness to travel locally for home care visits and attend onboarding in the Bronx

Company Information :

We are seeking a dedicated Nurse Practitioner Supervisor with strong clinical experience in Geriatrics, Primary Care, or Internal Medicine to join our team. The ideal candidate will bring at least 3 years of hands-on experience in one or more of these specialties and be comfortable with a mix of in-clinic and home care responsibilities.

Key Responsibilities :

Provide clinical supervision and leadership within the practice

Deliver patient-centered care in both the clinic and home settings

Conduct assessments, develop care plans, and coordinate treatment with interdisciplinary teams

Mentor and guide staff, ensuring compliance with clinical protocols and regulatory standards

Participate in quality improvement initiatives

Home Care Expectations :

• Queens-based candidates must be comfortable with approximately 20% home visits

Certifications :

DEA License and active Medicare & Medicaid billing IDs
