Career Opportunity : Substance Abuse Counselor – SUD / MAT Services. Location : Lincoln Heights - Cincinnati, OH. Organization : The Healthcare Connection. The Healthcare Connection is seeking a Substance Abuse Counselor to deliver high-quality clinical care for individuals with Substance Use Disorders (SUD) within a Medication-Assisted Treatment (MAT) program. This role involves providing individual, group, and family therapy, clinical assessments, treatment planning, and working collaboratively within a multidisciplinary team to support comprehensive care addressing both behavioral and medical aspects of substance use treatment.

Key Responsibilities

• Conduct comprehensive biopsychosocial assessments and diagnostic evaluations for SUD, co-occurring disorders, and related behavioral health concerns.
• Provide brief, solution-focused interventions and evidence-based therapies (e.g., CBT, Motivational Interviewing) to promote functional recovery.
• Facilitate individual, group, and family therapy sessions, addressing addiction, relapse prevention, and co-occurring mental health conditions.
• Engage in medication adherence counseling and self-management coaching alongside Addiction Medicine providers using a team-based care approach.
• Collaborate with primary care providers, case managers, peer support specialists, and addiction specialists to ensure coordinated, patient-centered care.
• Provide training and consultation to primary care teams regarding mental health, substance use, and behavioral medicine.
• Monitor and update individualized treatment plans in alignment with program goals and regulatory documentation standards.
• Manage crisis interventions, including suicide risk assessments and safety planning.
• Educate patients and their families on addiction, recovery processes, and mental health topics.
• Maintain thorough, accurate clinical documentation using Electronic Medical Records (EMR).
• Participate in quality improvement initiatives, staff meetings, and ongoing professional development related to SUD / MAT best practices.
• Identify and facilitate referrals to community resources and additional behavioral health services.
