Career Opportunity : Substance Abuse Counselor – SUD / MAT Services. Location : Lincoln Heights - Cincinnati, OH. Organization : The Healthcare Connection. The Healthcare Connection is seeking a Substance Abuse Counselor to deliver high-quality clinical care for individuals with Substance Use Disorders (SUD) within a Medication-Assisted Treatment (MAT) program. This role involves providing individual, group, and family therapy, clinical assessments, treatment planning, and working collaboratively within a multidisciplinary team to support comprehensive care addressing both behavioral and medical aspects of substance use treatment.

Key Responsibilities
- Conduct comprehensive biopsychosocial assessments and diagnostic evaluations for SUD, co-occurring disorders, and related behavioral health concerns.
- Provide brief, solution-focused interventions and evidence-based therapies (e.g., CBT, Motivational Interviewing) to promote functional recovery.
- Facilitate individual, group, and family therapy sessions, addressing addiction, relapse prevention, and co-occurring mental health conditions.
- Engage in medication adherence counseling and self-management coaching alongside Addiction Medicine providers using a team-based care approach.
- Collaborate with primary care providers, case managers, peer support specialists, and addiction specialists to ensure coordinated, patient-centered care.
- Provide training and consultation to primary care teams regarding mental health, substance use, and behavioral medicine.
- Monitor and update individualized treatment plans in alignment with program goals and regulatory documentation standards.
- Manage crisis interventions, including suicide risk assessments and safety planning.
- Educate patients and their families on addiction, recovery processes, and mental health topics.
- Maintain thorough, accurate clinical documentation using Electronic Medical Records (EMR).
- Participate in quality improvement initiatives, staff meetings, and ongoing professional development related to SUD / MAT best practices.
- Identify and facilitate referrals to community resources and additional behavioral health services.
Education and Qualifications
Minimum Qualifications :
Master's degree in Social Work, Psychology, Sociology, or a related field from an accredited program.Mental Health licensure in the State of Ohio (LCSW, LPC, LMFT, etc.) or eligibility for licensure.Licensed Chemical Dependency Counselor (LCDCII, LCDCIII, LICDC, LICDC-S) in Ohio.Work Experience
3+ years of experience in MAT-related behavioral health settings.Strong understanding of MAT principles and evidence-based treatments for co-occurring disorders.Proficiency with Microsoft Office and experience working with EMR systems.Knowledge of local community resources.Experience working with diverse patient populations.Core Competencies
Strong collaboration skills in integrated, multidisciplinary care teams.Ability to apply the biopsychosocial model to primary care and SUD treatment.Proficient in evidence-based therapeutic modalities (CBT, DBT, Motivational Interviewing).Knowledge of trauma-informed care, harm reduction, and culturally responsive practices.Effective problem-solving, organizational, and clinical documentation skills.Commitment to ethical standards, confidentiality, and professional conduct.Flexibility to work scheduled hours, including potential overtime.Preferred Qualifications
Experience working in addiction services, particularly MAT programs.Bilingual (English / Spanish) strongly preferred.Experience with NextGen EMR systems.Physical Requirements / Work Environment
