Job Summary : The Client Care Manager is responsible for building and maintaining meaningful relationships with older adults and their families while coordinating personalized care to support safe, comfortable, and dignified aging at home. This role serves as the primary point of contact for clients, overseeing the development and execution of customized care plans, leading care introductions, and conducting regular quality assurance visits. Key Responsibilities :
Client Engagement & Relationship Building : Serve as a trusted advocate for clients, ensuring they receive the highest standard of care and support.
Serve as the primary point of contact for clients and their families, building lasting, trust-based relationships.
Care Coordination & Quality Assurance : Oversee client service plans, ensuring that care is adapted as needs evolve.
Collaborate with the care team, schedulers, and Care Pros to ensure seamless service delivery.
Collaboration & Problem-Solving : Work closely with Care Pros, healthcare professionals, and family members to create the best outcomes for clients.
Address client concerns with empathy, professionalism, and proactive problem-solving.
Compliance & Documentation : Maintain accurate and up-to-date client records, including care assessments, service plans, and quality assurance documentation.
Adhere to all company policies, procedures, and compliance requirements.
