Summary : Enroll, coordinate services / referrals, and provide comprehensive nursing care via in-person and / or virtual visits to a caseload. Monitoring client vitals, recording changes in conditions, taking patient histories, and organizing health records. Visits may include nursing assessments, education, guidance, and support to families in their homes, locations of their choice, or via telehealth.

Duties and Responsibilities :

• Assessing patients : Home visiting nurses evaluate patients' well-being.
• Maintaining records : Home-visiting nurses maintain accurate records of the services they provide.
• Connecting with care providers : Home-visiting nurses collaborate with other providers and physicians.
• Monitoring vitals : Home visiting nurses monitor and record patient's vital signs, when necessary.
• Developing care plans : Home-visiting nurses help develop and maintain patient care plans.
• Documenting visits and submitting information within the specified time frame.
• Performs other related duties as assigned.
