💼 Job Duties & Workload

Medical discharges

Social Work Team : 15 FT + 17 PT Social Workers

UMMC contracts with 5 local facilities (10 beds each)

💻 Systems Used

🧑‍⚕️ Team Structure & Oversight

15 RN Case Managers (Goal : 18 general CMs + 3 complex CMs)
