6+ months of experience at a Level 1 trauma or large academic medical centerMust include medical discharge planningEPIC charting experience is requiredPreferred :CCM Certification (not required)
BSN preferred; ADN consideredPosition SummaryTitle : RN Case Manager β Discharge PlanningDepartment : Case Management β General / Discharge PlanningSetting : Inpatient β Covers all inpatient units throughout the hospitalShift : Monday to Friday, 8 : 00 AM β 4 : 30 PM (5x8hr shifts β non-negotiable )Weekend Requirement : NoneHoliday Requirement : Rotated as needed (β No time off allowed at Christmas )Orientation : 2 weeks (includes general facility orientation and department-specific training)π Location & Travel Rules
50-Mile Radius Rule : Applies to travelersLocal Candidates Accepted : Local pay rate is $68 / hrπ Requirements

Respiratory Therapy and Pharmacy : Consult as neededFacilities for Complex Discharges :UMMC contracts with 5 local facilities (10 beds each)

Current Team :15 RN Case Managers (Goal : 18 general CMs + 3 complex CMs)
