Job Summary :
The Population Health Strategy Program Lead designs, implements, evaluates and provides oversight to the Population Health Strategy through the integration of quality improvement science methodologies.
Essential Functions :
- Design Population Health Strategy that aligns with the state quality strategy and population needs.
- Execute clinical and operationalize tactical approaches for the Population Health Strategy program designs.
- Implement and operationalizes program design to meet the Population Health Strategy goals.
- Lead assigned Market's Population Health Committee.
- Lead the development, implementation and monitoring of action plans based on performance metrics (State Agencies, AHRQ, HEDIS, CAHPS)
- Complete a population health assessment on an annual basis or more frequently if required to align with NCQA and state requirements.
- Ensure Population Health Strategy design complies with NCQA and State requirements.
- Complete annual Population Health Strategy Evaluation in alignment with NCQA requirements.
- Develop and oversee Population Health policies and procedures.
- Develop and have oversight of data analysis reports
- Use statistical techniques, graphics and data to summarize HEDIS analytical needs to measure impact of various actions / results
- Evaluate and provide reports and findings regarding the processes of provider profiling, focus groups and stakeholder interviews
- Identify opportunities for improvement in clinical or operations performance measures and survey scores based on evidence-based results of quantitative and qualitative analysis to improve health outcomes
- Assist in the development of focus studies using epidemiological methodology to target population for interventions
- Contribute to the development of surveys, evaluation designs and facilitation guides
- Conduct process analysis to identify opportunities and develop solutions that can scale for reuse across other markets
- Investigate future best practices across various industries to bring to the work team
- Perform any other job related instructions as requested
Education and Experience :
Master of public health or related field degree (e.g. health promotion, evaluation etc.) is requiredMinimum of five (5) years of relevant work experience in program evaluation is requiredExperience using the model for improvement (PDSA-Plan, Do, Study, Act) is requiredPrevious healthcare or managed care experience is preferredCompetencies, Knowledge and Skills :
Proficient in Microsoft Word, Access and advanced proficiency in Excel is requiredKnowledge of a variety of quality improvement methodologies - Model for Improvement, Lean, Six SigmaKnowledge of establishing and documenting PDSA cycles and key driver diagramsAnalytic skills for solving problemsKnowledge of managed care and health care data coding is preferredGraphic development / presentation skillsFamiliar with descriptive statistical application and techniquesCritical listening, thinking skills, and verbal and written communication skillsProblem solving skillsExcellent writing skillsLicensure and Certification :
Six Sigma or project management certifications are preferredWorking Conditions :

$81,400.00 - $130,200.00 CareSource takes into consideration a combination of a candidate's education, training, and experience as well as the position's scope and complexity, the discretion and latitude required for the role, and other external and internal data when establishing a salary level. In addition to base compensation, you may qualify for a bonus tied to company and individual performance.
