job_description.job_card.job_descriptionInitiating contact with all Keystone patients living in home or assisted living environments as well as other patient / families as identified by the interdisciplinary team. Assessing the physical and emotional needs of patients, families and care givers related to the patient's life limiting illness environment. Providing support and education for patients, families and community members regarding symptom management, disease process, patient needs, emotional and social aspects of life limiting illness. Participating in goals of care discussions with patients and families as needed for patients identified as hospice appropriate. Assisting patients with medication refills and communicating symptom management concerns with provider and team. Graduate of an accredited school of nursing and current Registered Nurse (RN) licensure in the State of Michigan. Two Years of recent clinical experience is preferred. Homecare of hospice experience desired. Sound clinical judgment. Knowledge and / or experience in pain symptom management with end of life care. - Weekly Schedule Monday through Friday Hours 8 a.m. to 5 p.m.
- Salary commensurate with experience
Position Summary :
The palliative care Clinical Support Nurse (RN)works as a member of the interdisciplinary team telephonically assisting patients and families in managing the psychosocial, emotional and resource needs relating to life limiting illnesses.
