The Palliative Care Nurse Practitioner provides palliative care through the evaluation and assessment of the health history, health promotion, diagnosing medical conditions, management of health problems by directing and developing the plan of care, coordinating and collaborating with patients, families and other healthcare practitioners. Under the supervision of a physician, the nurse practitioner visits patients, completes physical assessments, performs face-to-face assessments and evaluates palliative care eligibility as assigned in consultation with the palliative care interdisciplinary team.

Responsibilities
Provide holistic assessments that look beyond physical health, addressing psychological, social, and spiritual needs.
Guide patients and families through meaningful goals-of-care conversations to ensure treatment aligns with their values.
Create and update personalized care plans in collaboration with patients, families, and the interdisciplinary team.
Manage pain and other symptoms with evidence-based strategies, balancing both medication and non-pharmacologic approaches.
Support safe transitions of care by reconciling medications and deprescribing when appropriate.
Initiate timely hospice referrals and ensure smooth handoffs when patients become eligible.
Document assessments, care plans, and advance care planning discussions thoroughly and accurately in the EMR.
Actively contribute to team meetings, case reviews, and quality improvement initiatives.
Share your expertise by mentoring colleagues and educating staff, referral sources, and community partners on palliative principles.
Stay current with clinical guidelines, regulatory requirements, and best practices to deliver the highest quality care.
Qualifications
Current, unencumbered licensure as a Nurse Practitioner or Physician Assistant in applicable state
Board Certification in applicable NP or PA specialty
Certification in Hospice and Palliative Care preferred (or willingness to obtain within 12 months)
Valid driver’s license and automobile insurance per policy
CPR Certification per state requirements
Master of Science in Nursing (MSN), Doctorate of Nursing Practice (DNP), or Master of Medical Science (MMSc / MPAS)
Minimum 1 year of experience in palliative care, geriatrics, or complex chronic disease management preferred
Strong knowledge of Medicare Part B, palliative care billing, and ICD-10 coding
Proficiency in EMR and mobile documentation workflows
Exceptional communication skills (verbal, written, and public speaking)
Ability to build trust quickly with patients, families, and clinicians while exercising sound clinical judgment
$100,00.00 - $110,000.00 per year (Average Pay Range). The pay range listed represents a general guideline for the role and is not a guarantee of the final offer. Compensation will be determined based on the selected candidate’s relevant experience and the specific responsibilities of the position. Final compensation rate will be discussed and confirmed at the conclusion of the interview process
