MDS / Nurse Manager - RN or LPN

Mascoutah Rehab and Nursing is seeking a dedicated MDS / Nurse Manager RN or LPN to join our leadership team in a full-time role. This position combines clinical leadership with responsibility for the MDS and care plan process, ensuring both high-quality resident care and accurate reimbursement.

Key Responsibilities :

Assist the Director of Nursing (DON) in overseeing day-to-day clinical operations.

Supervise, direct, and mentor nursing staff to ensure exceptional care delivery.

Coordinate, schedule, and complete all MDS assessments in compliance with federal and state guidelines.

Gather and analyze clinical data to develop accurate, individualized care plans.

Collaborate with interdisciplinary team members to ensure care plans reflect residents' needs and goals.

Monitor quality measures and contribute to ongoing improvement initiatives.

What We're Looking For :

Active state RN or LPN license required.

Prior experience in skilled nursing / long-term care required.

Solid understanding of the MDS process (coordination, scheduling, and completion).

Strong organizational and leadership skills with attention to detail.

Compensation & Benefits :

Salary :

$90,000 / year (RN)

$75,000 / year (LPN)

with opportunities for growth based on census

Quality Measure Bonus : Eligible to earn up to $7,500 annually .
