Lead APP role for Practice Expansion - Asheville, NC
Launched in 2022, At Home Harmony is an innovative home-based Medical and Pharmacy practice based in Richmond, VA expanding to Asheville, NC providing exceptional healthcare for aging, homebound adults with complex chronic conditions. Our team of dedicated medical providers is growing, and we're seeking an experienced, dedicated and forward-thinking NP / PA to join and help lead our expansion team in Asheville, NC .
As a Nurse Practitioner or Physician Assistant on our thriving team, and as the inaugural APP for AHHs practice in North Carolina, you'll play a pivotal role in our mission to provide exceptional healthcare services directly to patients with complex medical needs in the comfort of their homes. You'll help build our practice by helping build a local expansion team under the guidance of the Richmond-based leadership team. You will need an entrepreneurial skillset that allows for networking, building relationships and clinical operations oversight while providing clinical care. You will collaborate with our interdisciplinary team of physicians, pharmacists, nurses, MAs, phlebotomists, clinical liaisons, utilizing the latest tools and technology to enhance patient outcomes and streamline provider workflows.
Our current team of Nurse Practitioners and Physician Assistants bring a wealth of knowledge in primary care / internal medicine / palliative / hospice / geriatric experience and have all contributed greatly to our continued success in Richmond, VA. They each have an interest in relational, home-based primary care with the goal of caring for older adults with complex chronic conditions in their homes, group homes, and independent living.

Your Role :
Work with Richmond based leadership team to build and implement At Home Harmony clinical model in AshevilleRepresent AHH and help build relationships with local clinicians and hospital partners, as able (early aspect of role), while providing clinical care.Deliver medical care in the patient's home. This involves driving or riding in your vehicle to the place of serviceDiagnose, manage and treat complex chronic illnessesAnalyze test data to diagnose, treat and arrange appropriate follow up for the patient to ensure continuity of careAccurately and thoroughly document your patient encounter and ensure accuracyEducate patients and families on how to manage their health and well-beingEffectively navigate difficult conversations related to end-of-life issues and goals of careCommunicate effectively with patients, family, the medical power of attorney, other medical providers and all individuals involved in the patients careFollow At Home Harmony's Coordinated Model of Care of Medical and PharmacyUse critical thinking skills and follow evidence-based standards of practiceAdhere to clinical and safety standards, protocols, and performance metricsProvide care with compassion, empathy, and cultural competencyMaintain positive relationships with Care Concierge and At Home Harmony staffAttend training sessions and clinical team meetings, as requestedServe as ambassadors for At Home Harmony and maintain professional demeanorExperience and Qualifications :
Minimum of five years of experience, prefer 8 years or more, as an APP in Primary Care setting - Geriatrics, Family Practice, Internal Medicine, Palliative Medicine - with a passion for working with the Geriatric populationExperience and relationships with the Asheville medical communityTech savvy and knowledgeable with Microsoft Office, Athena EMR, AI, and other computer programs in a mobile settingExperience in home-based care a plus but desire to learn is welcomedStrong communication skills and willingness to perform business development and networking in addition to clinical dutiesIndependent, pro-active worker with leadership interest who also functions well in teams and can take guidance from remote leadershipGraduate with an advanced degree from an accredited institutionCurrent unrestricted NC state license as a Nurse Practitioner or Physician AssistantNationally board certified, currentCurrent BLS requiredActive Prescriptive authority and DEAValid driver's license with clean driving recordAbility to lift and carry equipment up to 50 poundsAbility to walk up and down several flights of stairs easilyCompensation Rate : Competitive base starting at $140,000 (based on experience); Incentive / bonus program; Health insurance, 401K with matching, Malpractice coverage, Cell phone allowance, Mileage reimbursement, Up-To-Date membership and numerous other benefits.
