Key Responsibilities :

• Implement and follow specific care plans for clients, documenting completed tasks.
• Assist clients with personal care activities, including bathing and dressing.
• Help with routine household tasks such as cleaning, meal preparation, and laundry.
• Provide transportation for clients to appointments and errands.
• Guide clients in self-administering medications.
• Monitor and report any changes in clients' conditions.
• Maintain confidentiality regarding clients' sensitive information.
• Demonstrate reliable attendance and flexibility to meet department needs.
• Adhere to Medicare, Medicaid, and HIPAA regulations.
• Comply with all company policies, procedures, and standards.
• Perform other related duties as assigned.
