About The Position

Certifications : RN Case Manager

Must-Have :

RN Case Management experience

Familiarity with MIDAS, Meditech, Care Port Discharge, Outlook, Excel, Interqual, and Word

Description :

Responsible for data collection and case management.

Perform assessments within one day of admission through face-to-face and phone interactions as needed.

Conduct Interqual reviews.

Maintain frequent liaison contact with home health, skilled nursing facilities, inpatient rehabilitation facilities, and long-term acute care.

Facilitate IV infusion, durable medical equipment referrals, and follow-ups.

Participate in daily pass down meetings.

Engage in multidisciplinary rounds.

Demonstrate flexibility and adaptability in a team-oriented environment.

Able to ask questions and receive direction.

Occasionally float to other areas for assistance.

Dress code is business casual or scrubs.

Work hours include a 30-minute lunch break.

Required to work every third weekend.
