We're Hiring : RN Assessor - IDD Services!

We are  seeking  Nurse Assessors  to join the  Independent Assessor (IA) Project , conducting both  in-home and telehealth assessments  for consumers applying for community-based long-term care programs.

Location : Manhattan, NY

Role : RN Assessor - IDD Services

Key Responsibilities :

  • Conduct assessments (telehealth or in-home) using the  Uniform Assessment System of New York (UAS-NY)  to evaluate consumer health, abilities, and long-term care needs.
  • Review medical documentation and referrals to ensure accurate, informed evaluations.
  • Maintain precise electronic documentation and properly code all UAS-NY data.
  • Educate consumers on healthcare options, managed care plans, and community-based resources.
  • Coordinate care and referrals with interdisciplinary teams.
  • Collaborate with the Clinical Quality Assurance Department to uphold compliance and quality.
  • Stay current on state regulations and best practices in long-term care.
  • Manage your own scheduling and consumer appointments to create a balanced, autonomous workflow.

    • What We're Looking For :

    Education & Licensure :

  • Graduate of an accredited nursing program (BSN preferred).
  • Active RN license in the State of New York.
  • Valid NY State ID or Driver’s License.

    • Experience :

  • 1–2 years of in-home care or community-based nursing preferred.
  • Experience in geriatric, home health, or chronic illness care is a plus.
  • Familiarity with UAS-NY assessments and related systems is highly desirable.

    • Skills & Requirements :

  • Strong interpersonal skills with the ability to build trust across diverse populations.
  • Self-motivated and highly organized, able to manage your own time and caseload.
  • Proficient in using electronic health records, UAS-NY systems, and telehealth platforms.
  • Reliable transportation required for in-person assessments.
  • Culturally sensitive, professional, and committed to quality care.
