We are seeking Nurse Assessors to join the Independent Assessor (IA) Project , conducting both in-home and telehealth assessments for consumers applying for community-based long-term care programs.

Key Responsibilities :
Conduct assessments (telehealth or in-home) using the Uniform Assessment System of New York (UAS-NY) to evaluate consumer health, abilities, and long-term care needs.Review medical documentation and referrals to ensure accurate, informed evaluations.Maintain precise electronic documentation and properly code all UAS-NY data.Educate consumers on healthcare options, managed care plans, and community-based resources.Coordinate care and referrals with interdisciplinary teams.Collaborate with the Clinical Quality Assurance Department to uphold compliance and quality.Stay current on state regulations and best practices in long-term care.Manage your own scheduling and consumer appointments to create a balanced, autonomous workflow.What We're Looking For :
Education & Licensure :
Graduate of an accredited nursing program (BSN preferred).Active RN license in the State of New York.Valid NY State ID or Driver’s License.Experience :
1–2 years of in-home care or community-based nursing preferred.Experience in geriatric, home health, or chronic illness care is a plus.Familiarity with UAS-NY assessments and related systems is highly desirable.Skills & Requirements :
