Advanced Practice Provider (NP or PA) – NJ Palliative Care

On-Site Role | Hospitals & Post-Acute Care | Active NJ License Required

NJ Palliative Care is expanding and seeking a dedicated Nurse Practitioner or Physician Assistant to join our interdisciplinary team. We proudly partner with RWJ Barnabas Health , Complete Care , and other healthcare leaders to deliver exceptional palliative services across New Jersey.

Practice Locations : Essex County

Hudson County

Union County

Middlesex County

Somerset County

Monmouth County

Ocean County

Key Responsibilities :

Provide direct palliative care in hospital and post-acute settings

Conduct patient evaluations, create and manage care plans, and adjust treatments as needed

Support and educate patients and families on care goals and options

Collaborate with physicians, nurses, and facility staff to ensure smooth transitions of care

Maintain accurate and compliant documentation of all patient encounters

Qualifications :

Active and unrestricted New Jersey NP or PA license (required)

Background in palliative medicine, geriatrics, internal medicine, or related specialty preferred

Strong communication, critical thinking, and interpersonal skills

Dedication to patient-centered, compassionate care

Compensation & Career Growth :

Team Lead positions available for highly experienced APPs with additional responsibilities
