Licensed Nurse Practitioner (LPN) - In-Home Health Risk Assessments (Part-Time, 3 Days / Week) Guaranteed Compensation : Earn $2100 semi monthly as a 1099 contractor. We're seeking a dedicated Licensed Practical Nurse (LPN) to perform in-home visits for patients across Hennepin, Anoka, Sherburne, Wright, and Stearns (St. Cloud) counties. This is a great opportunity for an LPN who enjoys working independently and values a flexible, part-time schedule. What We Offer : 2,100 per semi-monthly pay period ($350 / day) Flexible scheduling with a minimum commitment of 3 days per week Paid Training and mileage No on-call or follow-up care Company-provided iPad with a user-friendly documentation system (AthenaOne) Malpractice coverage and collaborating physician provided Your Role : As an LPN, you'll conduct comprehensive in-home HRAs for Medicare Advantage members. Each visit includes an evaluation of past medical history, review of systems, medication reconciliation, vital signs, and diagnostics labs and testing. No long-term care or follow-up is required. Requirements : Active, unencumbered MN LPN License. Willingness to travel within the specified counties. While not required, a provider open to occasional travel throughout the state is desired.
