🩺 About the Role

As a Licensed Practical Nurse (LPN) Care Manager , you’ll support members across Central Florida through telehealth check-ins, education, and coordination.

You’ll be their consistent point of contact — monitoring vitals, reviewing care plans, and connecting with their doctors to close care gaps.

You will :

• Manage an assigned panel of members (mostly Medicare / older adults).
• Conduct scheduled telehealth and phone visits to assess needs, coach on lifestyle and medication adherence, and identify early warning signs.
• Document findings, update care plans, and escalate issues to an RN or NP as needed.
• Coordinate follow-up appointments, lab work, and community resources.
• Collaborate with the Preemptive Health team to deliver measurable outcomes : better control, fewer hospital visits, happier members.
