Director of Patient Services

The Director of Patient Services (DPS) for a Certified Home Health Agency (CHHA) and Licensed Home Care Services Agency (LHCSA) is responsible for overseeing the clinical and operational aspects of patient services within the agency. This includes ensuring compliance with state and federal regulations, improving patient care, managing clinical staff, and maintaining the highest standards of service delivery. The DPS will collaborate closely with other departments to maintain operational efficiency, enhance patient satisfaction, and ensure the agency's success in delivering home health care services.

Key Responsibilities :

Clinical Leadership & Patient Care :

Oversee the coordination and delivery of patient care services, ensuring that all documentation and care plans meet the needs of each patient and are aligned with regulatory requirements.

Monitor and evaluate patient care services, ensuring adherence to agency policies and industry best practices.

Lead and supervise clinical staff, including nurses, therapists, home health aides, and other direct care providers.

Provide guidance and support to clinical staff on clinical issues, patient care concerns, and best practices for service delivery.

Regulatory Compliance :

Ensure the agency is fully compliant with all applicable state, federal, and local regulations.

Maintain and ensure proper documentation of all patient care activities, following agency policies and regulatory standards.

Prepare for and manage all audits and inspections by regulatory agencies, ensuring compliance is continuously met.

Quality Assurance & Improvement :

Implement and oversee quality assurance programs to monitor patient care outcomes, staff performance, and service delivery standards.

Develop and monitor key performance indicators (KPIs) to assess the agency’s patient care delivery and staff productivity.

Staff Management & Development :

Recruit, hire, train, and supervise clinical staff, ensuring that they are properly licensed, credentialed, and compliant with all relevant standards.

Conduct regular performance evaluations, providing feedback and guidance to staff on professional development.

Foster a collaborative and supportive work environment to ensure a high level of job satisfaction and retention among clinical staff.

Patient and Family Communication :

Serve as a point of contact for patient and family concerns, addressing complaints and resolving issues in a timely and professional manner.

Educate patients and families about their rights, care plans, and available resources.

Ensure that patients’ needs and preferences are respected in the development of care plans and service delivery.

Operational Support :

Collaborate with the agency’s leadership team to develop and implement policies and procedures that support the effective and efficient delivery of services.

Assist in budget development and financial planning for patient services, ensuring efficient resource allocation.

Provide leadership in emergency management, including disaster preparedness and response as it pertains to patient care.

Billing and Documentation :

Ensure that patient care documentation is accurate, timely, and compliant with payer requirements, including Medicaid, Medicare, and private insurance providers.

Work with billing and administrative teams to ensure proper coding and billing practices for all services rendered.

Qualifications :

Registered Nurse (RN) with a current New York State license.

bachelor’s or master’s degree in nursing, Healthcare Administration, or a related field preferred.

Experience :

Experience in home health care, with at least 2 years in a supervisory or leadership role.

In-depth knowledge of CHHA and LHCSA operations, regulations, and compliance requirements in New York State.

Proven experience in quality assurance, patient care coordination, and staff management.

Skills :

Strong leadership, organizational, and communication skills.

Ability to handle multiple tasks and prioritize in a fast-paced environment.

Proficient in inpatient care management and electronic medical records software. Knowledge of Medicare, Medicaid, and private insurance billing processes.

Work Environment :

The position involves a combination of office-based administrative work and field visits to assess patient care needs.

Flexibility to adjust hours may be required based on patient care needs and agency demands.
