Our mission is to deliver comfort, dignity, and support during every stage of the hospice journey. We are looking for a dedicated Weekend On-Call Hospice RN to join our team and help provide exceptional patient care.

Position Summary

The Weekend On-Call Hospice RN will be responsible for triaging patient needs, making home visits as necessary, and providing skilled nursing care during after-hours and weekend shifts. This role ensures continuity of care and timely response to urgent patient needs.

Key Responsibilities

  • Respond to patient and family calls during assigned on-call hours.
  • Assess patient condition and provide appropriate interventions.
  • Conduct home visits as needed for symptom management, medication administration, and urgent care needs.
  • Collaborate with the interdisciplinary team to coordinate ongoing patient care.
  • Document all visits and communications in accordance with agency, state, and federal regulations.
  • Provide emotional support and education to patients and families.

Qualifications

  • Current Texas RN license in good standing.
  • Minimum one year of hospice, home health, or related nursing experience (preferred).
  • Strong clinical assessment and critical thinking skills.
  • Compassionate and empathetic approach to patient care.
  • Ability to work independently and manage time effectively.
  • Reliable transportation and current auto insurance.

    • Schedule & Compensation

  • Weekend On-Call : Typically Friday 5 : 00 PM Monday 8 : 00 AM.
