JOB DESCRIPTION FOR ACT TEAM
POSITION TITLE : Program Assistant
DEPARTMENT : ACT Teams
RATE : $17 / hour
BROAD FUNCTION :
The Program Assistant is responsible for collaborating with the Team Leader to facilitate the day's operations in a supportive manner to both the team and clients. S / he maintains a professional, friendly, and welcoming environment in the office to for clients and staff. S / he provides direct support to staff, including monitoring and coordinating daily team schedules and supporting staff both in the office and field around clients' needs. S / he serves as a liaison between clients and staff to include centralized, office-based communication and coordination across ACT staff and clients to promote continuity of care. S / he actively participates in the Daily Team Meeting to contribute to reporting and recording of the recent ACT staff contacts with clients and their natural supports. S / he is responsible for maintaining program files including client records, office records, and other documents for filing and storage. S / he handles matters confidential in nature and must maintain confidentiality of all clinical, financial, personnel and proprietary information.
PRINCIPAL DUTIES AND RESPONSIBILITIES :
- The Team has Program Assistant (PA) available to facilitate the day’s operations in a supportive manner to both the team and consumers.
- The PA provides direct support to staff, including monitoring and coordinating daily team schedules and supporting staff in the office and field.
- PA serves as a liaison and staff, such as attending to the needs of office walk-ins and calls from consumers / natural.
- PA actively participates in the daily team meeting. The team member (s) providing PA on the team are engaged and contribute to the daily team meeting on a regular basis. They report on recent contact with consumers and natural supports in that meeting. They may also play a role in updating the log, Daily Staff Schedule, or other tools / paperwork related to planning program contacts.
Greets and welcomes clients and guests who are visiting the program office.
Answers office phone and routes calls and takes messages and returns calls for follow-up responses.Attends all Daily Team Meetings. Attends all other staff meetings as appropriate.Tracks all client contacts each month and submits to Clinical Supervisor and Director of ACT Services for monthly billingCompiles client financial records including allowance check stubs and receipts of purchases.Verifies Medicaid eligibility for each client.Assists with clerical office tasks such as typing, scanning, faxing, copying, and filing of documents.Hand out checks Recerts for Promise CareCompletes vehicle safety inspections and maintains vehicle service records. Inventories and maintains adequate office supplies.Documents to Finance DepartmentVitals for Highmark when RequestedUploading client’s files into Carelogic when given Certification from ProviderUpdates AmeriHealthKeep contacts left for BillingFace sheets page 1 and page 2, Carelogic updateSchedule Med Rep lunchesPetty Cash, Log Book and ReceiptsMaintains on call phone during day until staff-on-call picks it is and deliver next morning to Program AssistantSupplies, Maintenance for Office, CarsPA will send email to maintenance for any office related maintenance that is required on the team (lights, furniture and equipment moving)PA will monitor and keep a record before the team runs out.PA and Team Lead will work out a system to keep track of gas credit cards.Create and maintain daily log, schedule in Carelogic for each staff as assigned by Team Lead and the schedule can be change as need it. Staff can call to change the schedule.Quarterly Fire drillsPerforms other duties as requested, verbally or in writing.SCOPE, RPOSE, AND FREQUENCY OF CONTACTS :
