About the Phoenix Program : Bethany’s Black Hawk Phoenix Day Treatment Program is a specialized mental health program serving students with emotional and behavioral challenges across 14 Illinois school districts. In partnership with the Black Hawk Area Special Education District, the program combines academic instruction with an extended day of therapeutic services—including counseling, social-emotional skill building, and individualized treatment planning. The goal : to help students thrive at home, in school, and in their communities, with a focus on reintegration into their home districts and successful high school graduation.

Bethany for Children & Families is currently looking for a Case Manager in our Phoenix Day Treatment program! The Case Manager will implement individual counseling, family counseling, group counseling, social / emotional, recreational, leisure, educational, and case-management components of program services within the Phoenix Day Treatment Program. Ideal candidates must have an outgoing personality with the patience and desire to work with children with emotional problems between the ages of 8 and 21, and be able to maintain a flexible schedule in response to program needs or caseload assignments.

Base Salary - $52,416.00

  • For full-time employees in their first year of employment, one hour of vacation time is earned for every 26 hours paid , with a maximum of 80 hours earned per year . For full-time employees after their first year of employment, one hour of vacation time is earned for every 13 hours paid, with a maximum of 160 hours earned per year.
  • Full-time employees earn one hour of sick leave for every 21.67 hours paid.
  • Eleven (11) paid holidays ,
  • Agency contributions of 8% calculated on bi-weekly earnings to a 403(b) Thrift Plan ( retirement plan ) upon meeting plan eligibility criteria,

Case Manager job duties :

1. Maintain a service caseload as determined within the specific program plan by :

  • Assessing the appropriateness of requests for services and interpreting the agency’s services effectively to the client.
  • Formulating a preliminary and tentative assessment of clients’ functions and reviewing appropriate information with the Supervisor to develop an effective case plan / treatment plan commensurate with the clients’ needs.
  • Managing a workload which involves decision-making and accountability; using time in an organized and structured manner to provide services to clients assigned to his / her caseload and meet performance objectives.
  • Maintaining case records, submitting required reports, providing statistical data, etc., in accordance with the agency’s expectations.
  • Meeting regularly with the Program Supervisor to discuss clinical issues, issues surrounding service delivery, and critical issues requiring supervisory consent or referral source authority.
  • Developing and utilizing knowledge of agency and community resources appropriate to established case plans, and assessing the usefulness to individual client’s needs.
  • Being readily accessible to his / her clients for meetings according to their schedule or contract requirements at client-convenient times of the day or night.
  • Demonstrating a capacity and initiative to increase professional skills and knowledge.
  • Maintaining a professional relationship with peers and other service referral resources and providers.
  • Attending meetings related to agency services and / or staff training.
  • 2. Further the mission of the agency by actively participating in the accomplishment of the agency’s strategic plan.

    3. Perform other duties as assigned.

    Requirements :

    Case Manager Qualifications :

    1. Possess a minimum of a Bachelor’s degree in social work or a human services field :

    A. Preferred, a Bachelor’s degree in social work or a human services field and a minimum of five years experience, or

    B. A Master’s degree in social work or a human services field and two years of experience.

    2. Have the ability to fulfill all of the regulatory requirements for employees of Child Welfare Agencies as are prescribed in Illinois Administrative Rule and Procedure, including successful completion of the criminal background investigation, clearance from the Illinois CANTS, Health Care Registry, and Iowa Registry, and certification as a driver of children by the Illinois Department of Children and Family Services.

    3. Have the ability to provide own transportation in performing job requirements, including a valid driver’s license and acceptable insurance coverage.

    4. Meet the eligibility requirements for a case management position as assigned by the contracting state department of Iowa or Illinois.

    5. Maintain a flexible schedule in response to program needs or caseload assignments.

    6. Have sensitivity and responsiveness to cultural differences in the community.

    7. Effectively communicate orally and in writing.

    8. Be committed to the agency’s vision, mission, philosophy, structure, and goals with an ability to work as part of a treatment team.

    9. Be able to lift 30 lbs.

    10. Be able to carry car seats to and from the office and children’s home.

    11. Be able to physically attend to young children and infants.
