About the Phoenix Program : Bethany’s Black Hawk Phoenix Day Treatment Program is a specialized mental health program serving students with emotional and behavioral challenges across 14 Illinois school districts. In partnership with the Black Hawk Area Special Education District, the program combines academic instruction with an extended day of therapeutic services—including counseling, social-emotional skill building, and individualized treatment planning. The goal : to help students thrive at home, in school, and in their communities, with a focus on reintegration into their home districts and successful high school graduation.
Bethany for Children & Families is currently looking for a Case Manager in our Phoenix Day Treatment program! The Case Manager will implement individual counseling, family counseling, group counseling, social / emotional, recreational, leisure, educational, and case-management components of program services within the Phoenix Day Treatment Program. Ideal candidates must have an outgoing personality with the patience and desire to work with children with emotional problems between the ages of 8 and 21, and be able to maintain a flexible schedule in response to program needs or caseload assignments.
Base Salary - $52,416.00

Requirements :
Case Manager Qualifications :
1. Possess a minimum of a Bachelor’s degree in social work or a human services field :
A. Preferred, a Bachelor’s degree in social work or a human services field and a minimum of five years experience, or
B. A Master’s degree in social work or a human services field and two years of experience.
2. Have the ability to fulfill all of the regulatory requirements for employees of Child Welfare Agencies as are prescribed in Illinois Administrative Rule and Procedure, including successful completion of the criminal background investigation, clearance from the Illinois CANTS, Health Care Registry, and Iowa Registry, and certification as a driver of children by the Illinois Department of Children and Family Services.
3. Have the ability to provide own transportation in performing job requirements, including a valid driver’s license and acceptable insurance coverage.
4. Meet the eligibility requirements for a case management position as assigned by the contracting state department of Iowa or Illinois.
5. Maintain a flexible schedule in response to program needs or caseload assignments.
6. Have sensitivity and responsiveness to cultural differences in the community.
7. Effectively communicate orally and in writing.
8. Be committed to the agency’s vision, mission, philosophy, structure, and goals with an ability to work as part of a treatment team.
9. Be able to lift 30 lbs.
10. Be able to carry car seats to and from the office and children’s home.
11. Be able to physically attend to young children and infants.
