Associate Marriage and Family Therapist
Location : San Leandro, CA
Schedule : Monday – Friday, 8 : 00 a.m. – 5 : 00 p.m.
Salary Range : $66,000 – $80,000
Position Summary
We’re looking for an Associate Marriage and Family Therapist (AMFT) to support our Integrated Behavioral Health Program in San Leandro, CA. This role provides an opportunity to gain real-world experience in an interdisciplinary setting that bridges mental health and primary care. You’ll receive direct clinical supervision while helping families access the care they deserve.
Key Responsibilities
Care Coordination & Program Support
- Deliver in-person and telehealth-based care coordination for individuals with behavioral health and social support needs
- Conduct comprehensive client assessments and contribute to individualized care planning
- Facilitate warm handoffs and referrals to behavioral health, medical, housing, and community-based resources
- Monitor engagement and support follow-up care to maintain service continuity
- Collaborate with internal care teams and external partners to address client needs holistically
- Participate in multidisciplinary team meetings, case reviews, and service planning sessions
- Document services and progress accurately in the electronic health record (EHR)
- Assist with data collection and service tracking to meet reporting requirements
Compliance & Training
Ensure compliance with Medi-Cal, HIPAA, and internal documentation protocolsAttend required trainings, community forums, and IBH-related meetingsSupport program evaluation activities by monitoring outcomes and service deliveryContribute to outreach, resource navigation, and sustainability effortsRequirements
Registered Associate Clinical Social Worker (ASW / ACSW) in the State of CaliforniaBachelor’s or Master’s degree in Social Work, Psychology, Public Health, or a related fieldMinimum 1 year of relevant experience in behavioral health, case management, or social servicesFamiliarity with Alameda County systems of care, including Medi-Cal and community-based resourcesUnderstanding of trauma-informed and culturally responsive service deliveryStrong communication, time management, and interpersonal skillsAbility to work collaboratively with both clinical and non-clinical team membersPreferred :
Experience in integrated or primary care settingsProficiency in an electronic health record system (SmartCare or similar)Bilingual skills (Spanish or other languages) highly valuedReliable transportation and valid CA driver’s licenseWork Environment
Hybrid community-based and clinic-based settingOccasional travel within Oakland CAUse of standard office and telehealth equipmentMay require occasional evening or weekend flexibilitySupervision & Support
