Minimum of 2 years of experience as an inpatient Registered Nurse Case Manager.

Complete a comprehensive patient assessment, including evaluating patient status to facilitate discharge.

Drive progression during the patient's stay.

Plan for discharge in a manner that expedites the patient’s transition to the next level    of care.

Maintain comprehensive and accurate documentation in EPIC .

Ability to identify necessary tasks and follow through to completion without prompting.
