Role Description

This is a full-time on-site role for a Hospital Liaison located in Wichita, KS. The Hospital Liaison will be responsible for acting as the primary point of contact between the hospital and Advena Living communities. Key duties include building and maintaining relationships with hospital staff, coordinating patient transitions from the hospital to Advena Living facilities, and ensuring seamless communication and documentation. The Hospital Liaison will also be involved in developing and implementing strategies to improve patient care and satisfaction, and supporting marketing and outreach efforts to increase community awareness of Advena Living services.

Qualifications

Strong relationship-building and interpersonal skills

Experience in care coordination and patient transition management

Excellent communication and documentation skills

Ability to develop and implement strategies for improving patient care

Marketing and outreach experience is a plus

Bachelor's degree in Nursing, Healthcare Administration, or related field

Experience in long-term care or skilled nursing facilities is beneficial
