Position Title : Registered Dietitian – PACE Program
About the Opportunity :
We are partnering with a well-established and mission-driven PACE (Program of All-Inclusive Care for the Elderly) provider in the Greater Louisville area to identify a compassionate and experienced Registered Dietitian . This is a direct hire opportunity —you will be joining a collaborative interdisciplinary care team dedicated to improving the health and quality of life for frail seniors who wish to remain in their communities.

Key Responsibilities :
Conduct comprehensive nutritional assessments for PACE participants.
Develop and implement individualized nutrition care plans based on participant needs, medical diagnoses, and cultural preferences.
Collaborate with physicians, nurses, social workers, and other interdisciplinary team members to coordinate care.
Provide nutrition counseling and education to participants, caregivers, and staff.
Monitor participant progress and adjust nutrition plans as needed.
Ensure compliance with regulatory and PACE-specific guidelines (CMS, state, etc.).
Support food service operations and menu planning in conjunction with kitchen staff (if applicable).
Participate in team meetings, quality improvement activities, and staff training.
Qualifications :
Bachelor's degree in Nutrition, Dietetics, or a related field (Master’s preferred).
Current Registered Dietitian (RD / RDN) credential through CDR (Commission on Dietetic Registration).
Licensed Dietitian / Nutritionist in the state of Indiana (or ability to obtain upon hire).
Minimum of 1–2 years of experience working with elderly or chronically ill populations —PACE, long-term care, home health, or geriatric settings preferred.
Strong interpersonal and communication skills.
Ability to work independently and as part of a collaborative team.
